Recognizing Near-misses For several years the Eagle Point Refinery has tracked near-miss incidents to help prevent future incidents and correct potential hazards. In late 2008, the refinery’s Health and Safety Committee (H&SC) began the Safe Worker Recognition Program to identify and reward employees and contractors submitting high quality near miss reports.
Near miss reports are reviewed daily and high quality reports are marked. The marked reports are then reviewed monthly by the H&SC. The committee uses the following questions to stimulate discussion:
- Which near miss was the most surprising?
- Which hazard occurs the most?
- Which hazard within the near miss could have been the most significant outcome?
- Which near miss had the most effective corrective action?
- Which near miss would you consider the “best” near miss and why?
The best report is selected and the author is named the Safe Worker of the Month, their near miss report is reviewed at the HES Monthly Meeting, and awarded a gift card for Sunoco gasoline. |
